Associate Benefits

Condell Medical Center offers a generous benefit package to associates. In order to be eligible for all benefits, associates must be scheduled to regularly work at least 40 hours per pay period or 20 hours per week. Some benefit programs may be available to other classifications of associates (as specified).

Health Insurance

Benefit eligible associates have a choice between our two PPO Plans administered by Humana.

Health insurance becomes effective the first of the month after 30 days of employment in a benefit-eligible position.

Dental Insurance

Benefit eligible associates have a choice between our PPO Indemnity Plan or Cigna DMO dental insurance.

Dental insurance becomes effective the first of the month after 30 days of employment in a benefit-eligible position.

Vision Insurance

Benefit eligible associates may participate in our Vision Insurance program, offered by Superior Vision.

Vision insurance goes into effect the first of the month following 30 days of employment in a benefit-eligible position.

Disability Insurance

Condell provides full-time and full-time modified associates with short and long term disability coverage, at no cost after six months of employment as a full-time associate.

Short-term disability coverage provides:

Fifty percent of salary from the 61st day of disability to the 180th day for associates with six months to five years of service.

Sixty percent of salary from the 61st day of disability to the 180th day for associates with five to ten years of service.

Seventy percent of salary from the 61st day of disability to the 180th day for associates with more than ten years of service.

Long-term disability coverage provides:

Sixty percent of salary from the 181st day of disability to a maximum length of time specified in the insurance contract.

Paid Time Off

Paid time off (PTO) is a bank of time to be used for vacations, personal business, time off needed to tend for sick children or other family members or your own personal illnesses (up to three consecutive days).

PTO time accrues at a level based on position level, full-time equivalency and years of service.

Accruals begin on your first day of employment and may be used after completion of three-months of service in a benefit-eligible position.

Holidays

Condell offers six paid holidays (based on associate's full-time equivalences) for full and part-time benefit eligible associates in observance of the following holidays:

New Year's Day

Memorial Day

Fourth of July

Labor Day

Thanksgiving Day

Christmas Day

Two floating holidays are also available to benefit eligible associates to use at your discretion.
Holiday accruals begin on your first day of employment.

Extended Illness Time

Extended Illness Time (EIT) is to be used for time off due to an associate's own personal illnesses or injury and is paid beginning the fourth consecutive day of absence.

Full-time associates accrue eight EIT days per year (part-time, benefit eligible associates accrue a pro-rated amount based on their full-time equivalencies).

Accruals begin on your first day of employment and may be used after completion of three-months of service in a benefit-eligible position.

Life Insurance

Condell Medical Center provides full-time and full-time modified associates with life insurance coverage, at no cost after six months of employment in an eligible status.

Full and part-time benefit eligible associates have the opportunity to purchase supplemental life insurance policies for themselves and their dependents at discounted rates.

401(k)

Condell Medical Center's 401(k) is administered by M & I Institutional Trust Services.

New associates are enrolled in the 401(k) the first of the month after 45 days of service.

All associates 18 years of age and older are auto-enrolled at a 3% deferral rate.  Associates that choose not to participate will need to opt out of contributions to the 401(k).

Condell Medical Center will match $.50 to every $1.00 deferred up to the first 3% contributed.  Maximum annual contributions for 2008 are $15,500 or $20, 500 if the participant is over the age of 50.  You must be vested (completed three years of service at a minimum of 1,000 hours worker a year) to receive the employer match.

Condell Medical Center will make a Safe Harbor Contribution of 3% to all associate's accounts, regardless of participation in the 401(k) at the end of each plan year.

Flexible Spending Account

Regularly scheduled associates, working at least 16 hours per week are allowed to participate in flexible spending accounts as a tax-effective way to pay for unreimbursed medical expenses and/or dependent care.

$3,000 annual maximum for unreimbursed medical

$5,000 annual maximum for dependent care

Coverage must be elected within an associates first 30 days of employment in an eligible status.

associates must re-enroll for participation in the following year during designated enrollment periods.

Supplemental Insurance Coverage

A variety of supplemental insurance coverage is offered to Condell associates at discounted rates.

Tuition Programs

Tuition Reimbursement

Full and part-time, benefit eligible associates (at least 40 hours per pay period) are eligible to continue their education by participating in our tuition reimbursement program.

associates are eligible to participate after completing six months in an eligible status.

Full-time associates can receive up to a maximum of $3,000 per calendar year.

Part-time associates can receive up to a maximum of $1,500 per calendar year.

Payments are for tuition charges only, up to a maximum of two classes per semester.

Educational Assistance

Condell's Educational Assistance program provides pre-paid tuition to attend classes at the College of Lake County (or other facilities as designated by the Medical Center) in specific "targeted career fields" only. Full and part-time associates (at least 16 hours per pay period) are eligible to apply for admission in our Educational Assistance program.

Targeted career fields are as follows:

Phlebotomy

Radiology Technologist

Registered Nursing

Surgical Technician

To be eligible, programs of study must be completed within time frames specified by the Medical Center.

Olivet BSN Completion Program

Condell and Olivet Nazarene University have partnered to offer an on-site BSN Completion Program to Associate Degree Nurses interested in returning to school to complete their nursing education.
Tuition for this program is paid at 100 percent by Condell Medical Center! Condell agrees to prepay $3,000 of tuition charges and then reimburse the remaining balance of tuition charges only at the successful completion of each term.

associates must be full or part-time benefit eligible to participate in this program.

Adoption Assistance Program

Condell Medical Center provides financial support to associates seeking to expand their families through adoption.
Full and part-time benefit-eligible associates who have been continuously employed for at least one year are eligible to participate in this program.

Associate Assistance Program

All Condell Medical Center associates and their immediate, dependent family members are eligible to participate in our Associate Assistance Program (AAP) for referrals and/or short-term counseling for personal issues that affect their daily lives.
