Whether patients are recovering from illness or experiencing their last stages in life, Condell Home Services helps facilitate the transition from hospital to home and from home to wellness.

Condell Home Services provides comprehensive skilled health care to the community on a referral basis and focuses on the patient to provide safe, compassionate, individualized care in a variety of settings.

Because every patient's needs are different, our interdisciplinary assesses and educates each patient on how to function best at home. Through its "one call does it all" policy, Condell Home Services coordinates the services of its four departments with the patient, the physician and family to provide comprehensive communication among those responsible for the patient's health and well-being.

Home Health Care: Provides intermittent, skilled nursing services along with physical, speech and occupational therapies, medical social work services and certified nursing aides for patients recovering from illness at home.

Home Medical Equipment: Provides medical equipment to patients at home. Services include selection, delivery, setup, instruction and maintenance.

Hospice: Provides palliative and supportive care for individuals in the final stages of a terminal illness and emotional support for the friends and family.

Home Pharmacy: Provides and delivers prescribed medications, supplies and devices to the patient's residence as ordered by the physician.

Accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and Medicare/Medicaid certified, Condell Home Services is a contracted provider for managed care organizations and all staff members of Condell Home Services are professionally licensed or certified.
