Condell Hospice

Condell Hospice provides palliative and supportive care at home for terminally ill patients and their families. Hospice is usually considered when one's life expectancy is approximately 6 months or less and the patient wishes to stay at home to spend the remaining days in the comfort of familiar surroundings.

Condell Hospice seeks to enable patients to manage their pain and symptoms so that their last days may be spent with dignity and quality at home.

Upon referral from a physician, our intake department verifies insurance and benefits. Hospice staff members will then discuss the program with the patient and family. The Hospice Team of professionals includes a medical director, registered nurses, a Chaplain and medical social worker (MSW), certified nurses' aides and trained volunteers. All of these members work to support not only the patient, but the entire family.

Condell Hospice prides itself on the quality of care and outstanding customer service it provides to each patient. Accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Condell Hospice is a contracted provider for managed care organizations and is Medicare/Medicaid certified.
