How does Condell Home Pharmacy work?

After a referral from your doctor, our intake department gathers necessary information from you. Your home pharmacy services are then easily coordinated with Condell Home Health Care, Hospice and Home Medical Equipment. The Pharmacist then clinically verifies and monitors medication orders and dispenses medication. Condell Home Pharmacy then delivers medication to the patient's home or residence.


Who pays for home pharmacy services?

Coverage for medication or supplies prescribed by a physician, will be verified by our intake department. Medicare and Medicaid and most insurance companies now offer policies that include coverage of expenses for prescriptions.

What are the qualifications of the Condell Home Pharmacy staff?

Condell Home Pharmacy is staffed by registered pharmacists and service technicians trained by Condell Medical Center.

How will I know when to order more supplies?

A nurse from Condell's Home Services will assist you in ordering more supplies. Phone 1-800-813-5185

How will I know when my therapy is done?

Your physician determines when your therapy is finished based on your progress. This is usually determined by a physician's exam. That determination is then communicated to the patient and/or caregiver.

Where do I store my infusion medication?

Most, but not all infusion medication should be kept in your refrigerator until 30 minutes before administering. This helps prolong the stability of the medication. Storage information is located on the upper right side of our pharmacy labels.

How are emergencies handled?

Please call 9-1-1 if your requirements extend beyond the capabilities of Condell Home Pharmacy staff.
