Cardiac Rehabilitation And Support
Condell's Cardiac Rehabilitation Program offers three phases to help patients achieve optimal health. Our professional team designs an exercise and education program that fits each patient's lifestyle and personal goals. The patient's family is also encouraged to take an active role in the patient's rehabilitation process.A cardiologist directs the program, which is administered by registered nurses, exercise physiologists, educators and dietitians. The Outpatient Cardiac Rehabilitation Program is conveniently located at the Centre Clubs in Libertyville and Gurnee. Condell Day Center vans can provide transportation to the Centre Clubs or any other Condell facility, 6:30 am to 6 pm Monday - Friday. $12 round-trip. Call us (847) 990-5828.

Three Phases of Cardiac Rehabilitation
Phase I- Begins in the hospital after a heart attack, open heart surgery or angioplasty
- Teaches patients and families about exercise, diet and controlling risk factors
- Guides and encourages patients to sit up and take a few steps as soon as possible after surgery
- Helps the patient develop an exercise plan to use when returning home
- Begins following surgery or angioplasty after discharge from the hospital
- Involves exercise at Centre Club for an hour, three times a week
- Monitors patients with special equipment by healthcare professionals
- Educational classes on cardiovascular disease, diet and stress management
- A less structured exercise program that promotes ongoing cardiac improvement and maintenance
- Is designed to foster continued motivation and supervised exercise for cardiac patients
